Care Designed Around Your Needs

Our goal is to help you regain or retain your independence and continue to have choice and control in how you live, and how you are supported – all within the comfort of your own home. From our first initial introduction, we work with you to design a personalised care plan so that you receive exactly the support you need.

Your Personalised Care Plan

Your individual support plan will look different for each person. We follow a careful process to create a tailored support plan that meets your needs, preferences and goals. From initial introductions, to building your personalised care plan and matching you with just the right care professional, the entire process is shaped around you. If you have been referred under ACC, there will be specific inclusions that you are funded for.

Care Planning

Setting and working towards short and long term needs, or goals, such as greater strength, mobility, confidence and independence

Private Home Care

Private home care can be arranged that is bespoke to your situation. Private care is self-funded, and can be anywhere from a few hours, to full time live-in care. Give us a call to chat about the options and organise an obligation-free consultation.

ACC Home Care

We are an approved provider under the ACC Home & Community ‘Maximise independence’ category, offering support for clients who have experienced the impact of a life-changing injury. This means we can provide comprehensive care for people with complex medical needs, such as spinal injuries, brain injuries, and high-needs rehabilitation.

ACC home care services are customised to meet the requirements of your ACC assessment. Whether it’s just a little helping hand, or more extensive daily care and medical support that is required, we’re here to help, with a wide range of specialist services to help you recover or manage short­ term, long-term, or complex injuries.

ACC Approved Hours

Our home care services are fully-funded by ACC up to the approved number of hours and scope of services that you have been assessed for, so there is no cost to you, unless you request additional services. You will need ACC assessment and a referral to qualify before organising care. Talk to your GP or health provider for more information, or call us if you would like advice on next steps.

Next steps

ACC Clients

If you have been approved for ACC Care (normally through your GP or medical provider following an injury), and chosen us as your care provider, it’s easy to organise funded care. Just get in touch and we’ll set up an initial meeting to talk to you about your needs and begin work on your tailored support plan. Or you can discuss with your ACC coordinator and they will put you in touch with our team.

Private Clients

In Home Support can be arranged privately for customers who are self-funding. Just get in touch to talk about bespoke options for support.
